            Time component has a critical role in both positive & negative ways on an urgent intervention to a patient or an operation or providing a critical product like blood. A blood reaching in time may save a life on the other hand, a little intervention that passes time may cause a loss of life. Time can work for surviving, happiness on one side & on the contrary may work for sadness, sorrow & death. Misusage of time source in health institutions & hospitals causes non removable materialistic & moral loss for patients & public. Lost machine, money like sources can be recompensed, however it is not possible to bring back a human whom has lost the life. However in our hospitals it is seen that this time which is assumed to be similar to money & wished to be beneficial is not properly used.

            For the best evaluation of time source; it is necessary to make a careful planning in, operating halls of hospitals & other health institutions must be used in an optimum way, the expiration days of the drugs in pharmacies, secure stock determinations considering the ordering periods, patient s queue waiting periods in policlinics or laboratories, sterilization unit set preparation actions & time etudes, etc.

            Some of the leading duties of Professional Health Administrators & Modern Hospital Administrators are; managing time, managing changes, managing study, managing information, in short, managing institutional events & related production sources. According to USA criteria each patient that admitts to hospital at least passes two working days in hospitals. There is no criteria in Turkey on this subject, only my & Doctor AKAR's Prime Ministry TODAIE Kaya Project  scoped study in Ankara Numune Hospital gives information. According to this study an ENT Patients passed time from arriving to hospital policlinic till obtaining the prescription; due to work flow, activity & time etudes are made, patient spent two working days in hospital for a policlinic inspection & in return to this obtained only a health service of seven minutes from the hospital (inspection & analysis).

            If we consider USA criteria, in other words a loss of two working days for each application, our work day loss becuase of application to policlinics will be 246.228.276 working days. When observed form point of General & Medical Sociology, Medical Antropology & Patient, Patient relative behaviour in Turkey, it is seen that none of the patients admitt to a hospital alone, at least with an accompany, spouse, children, relatives. This behaviour increases our work power loss to four times more. Even when this behaviour is considered to increase work day loss twice only hospital policlinics application caused work power loss will be 492.456.552.

            Besides except hospitals, there are 544.249 Home Births, 2.324.521 Tuberculosis Fight Policlinics, 2.035.254 VSD Microfilm production, 71.225.376 Village Health Policlinics, 616.709 Minor Intervention in Village Health Clinics, 2.388.555 MCH/RP pregnant tracking, 4.549.579 baby inspections, 5.233.022 child inspections of total 86.882.013 Health process is a fact. If we assume two working days for each admission 173.764.026 work day loss is foumd. When number of hospitalized days, inspections in hospital policlinics & other health institutions inspections & other health institutions activities related work power loss are considered total recorded work power loss will be 698.164.190. When unrecorded health admissions are considered, the dimension of the work power loss will increase.

            Lets assume that health institutions application causes a work power loss of 700.000.000 & for that year this work day loss by minimum wage (306.000.000 TL) corresponds to 214 Catrillion TL. ıf that terms Government budget is 97.8 Catrillion, Ministry of Health budget is 2.3 Catrillion TL, Health services application caused cost is 2.2 times more than Government Budgets & 94 times more than Ministry of Health Budget. Calculated work power loss cost in terms of new determined minimum salary (487.350.000 TL) is 341 Catrillion TL. or with YTL account, 341 Milyar YTL.

            With an effective & integrated HIMS in Health institutions, the mentioned Health institutions have a % 20 less admission caused work power loss; & with this foreseeing time loss profit will be ( 700,000,000 x % 20 ) 140,000,000 working days,  today evaluation (487,350,000 TL.) financial amount is 70 catrillion or 70 Billion YTL. (Turkey's Health expenditure is approximately 20 Catrillion TL due to the Health  Expenditure  Research of the Ministry of Health)

            This sample condition of health economy shows the necessity of the reconstruction of the Health System must be urgently desiged due to system properties according to reconstruction of Turkey, necessary to be left to Modern Health Administrators & Professional Hospital Administrators management.
